Individual Therapy
Our highly trained team of behavioral health clinicians practice a trauma-informed model of care to provide one on one therapy with individuals struggling with behavioral health, substance use or co-occurring disorders. The goal of our clinicians is to empower the individual to build and refine the skills necessary to grow in recovery. Apex’s Behavioral Health team is highly qualified & experienced working with the LGBTQ+ community to assist them with the additional barriers faced to lead a fulfilling life within their family and community.
PEER RECOVERY COACH
Our peer recovery coach will walk side by side with individuals seeking recovery from substance use disorders. Our recovery coach will assist every individual with their customized recovery plan and help them develop action steps toward their recovery goals. The recovery coach will assist in referrals for detox and residential treatment programs based on everyone’s needs. LATINO OUTREACH CASE MANAGER
Our Latino Outreach Case Manager provides culturally enhances case management services for Latinx individuals with substance use or co-occurring disorder or those at risk for such disorders. The Latino Outreach Case Manager spends most of their time engaging within the community to help eliminate barriers to accessing care and services for the Latinx community. Some of the Latino Outreach Case Manger’s responsibilities include arranging transportation to appointments, reducing stigmas by providing education and raising awareness of substance use issues within the Latinx community, assuring that the Lantinx community feels supported while locating and following through with services necessary for a better quality of life. BEHAVIORAL HEALTH CASE MANAGEMENT
Our behavioral health case management services focus on individuals with behavioral health and/or substance use disorders. The case management team will work directly with your clinician and assess the individual needs of each client and will assist with additional support and assistance obtaining resources within the community.
The focus of the case management team is to collaborate with the individual to identify goals, specific needs and work collaboratively to achieve these goals by connecting the individual with community resources, assist with housing needs, arrange transportation to appointments, assist in the enrollment or state benefits, assist with resume building, employment searches, and a variety of other skills and needs. The overall goal of our case manager is to assist the individual in improving their independence and function in their daily life.
